GLP-1 Receptor Agonists: Current State and Future Directions in Obesity Treatment
Current State
GLP-1 receptor agonists (GLP-1 RAs) are well-established for obesity treatment, leveraging their effects on glycemic control, gastric emptying, and appetite regulation (Nauck et al., 2017; Zhao et al., 2021). Semaglutide and liraglutide are prominent examples, with development efforts focused on extending half-life and improving delivery (Knudsen et al., 2019; Nauck et al., 2020). Dual agonists, such as tirzepatide (a GIP and GLP-1 RA), have shown superior efficacy in type 2 diabetes and are being investigated for obesity (Coşkun et al., 2018; Frías et al., 2021). Oral formulations like orforglipron represent a significant advancement in patient convenience (Wharton et al., 2025).
Strongest Evidence
Clinical trials consistently demonstrate significant weight loss with GLP-1 RAs. Semaglutide has shown efficacy in reducing cardiovascular events in patients with obesity and established cardiovascular disease, even without diabetes (Lincoff et al., 2023). Beyond weight loss, GLP-1 RAs exhibit broader metabolic benefits, including potential for treating nonalcoholic steatohepatitis (NASH) (Newsome et al., 2020). The mechanism involves widespread GLP-1 receptor distribution beyond the pancreas, influencing neuroinflammation and nerve growth (Zhao et al., 2021).
Unresolved Uncertainties
While generally safe, rare adverse events associated with GLP-1 RAs, such as suicidality and nonarteritic anterior ischemic optic neuropathy, require further investigation (McIntyre et al., 2023; Abbass et al., 2025). Long-term efficacy and safety data, particularly for newer compounds and oral formulations, are still accumulating. Optimal patient selection criteria and the comparative effectiveness of different GLP-1 RAs and dual agonists in diverse patient populations remain areas of ongoing research.
